Job summary

Constant Care Health Services LLC is seeking an Occupational Therapist (OT) to provide skilled therapy in a Bronx, New York home health setting. You will design and implement customized therapy plans to help patients regain independence and improve quality of life.

Qualifications

• OT certification and New York state licensure required.
• Bachelor's or Master's degree in Occupational Therapy.
• Experience in skilled nursing facility setting preferred.

Responsibilities
