Metro Physical & Aquatic Therapy in Riverhead, NY is seeking a Full-Time Occupational Therapist to join our Homecare team. You will provide 1:1 therapy in patients' homes, evaluate and plan individualized treatment, and educate families on proper therapy techniques.

Qualifications

• Licensed Occupational Therapist (OT) in NY or eligible.
• OT program graduate with licensure or eligibility.
• Experience in home health is preferred, not required.

Responsibilities

• Treat patients within your designated geographic area.
• Provide one-to-one therapy sessions.
• Evaluate patients and develop individualized treatment plans based on orders and needs.
• Deliver care that aligns with standards, policies, and the prescribed plan of care.
• Educate family members and caregivers on therapy techniques.
• Plan and carry out OT programs to maintain or improve function and relieve pain.
• Perform initial exam and determine diagnosis prior to intervention.
• Document visits timely and accurately.
• Adjust treatments based on patient progress to maximize benefit.
• Willing to treat geriatric patients.
