Job summary

Jukebox Health, Inc. is seeking an Occupational Therapist to perform in-home assessments and provide personalized recommendations for home safety and accessibility. The role supports older adults and individuals with disabilities to live independently at home.

The successful candidate will be empathetic, organized, and capable of collaborating with a remote team while maintaining timely, accurate documentation in our electronic system.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Active, unencumbered OT license.
• 2+ years of clinical experience.
• Experience in home health, aging in place, or disability services preferred.

Responsibilities

• Conduct in-home assessments and functional evaluations.
• Deliver personalized, evidence-based recommendations for home modifications.
• Educate clients and caregivers on fall prevention and safe mobility techniques.
• Engage clients and families in collaborative care planning.
• Work with Director of Clinical Operations to share findings and support process improvement.
