Job summary

The contract position requires an active OT license, 2+ years of clinical experience, and reliable transportation. You will collaborate with a remote clinical operations team and be paid monthly per completed evaluation.

Qualifications

• OT license active and unencumbered.
• 2+ years of clinical experience.
• Experience in home health, aging in place or disability services preferred.
• Ability to work independently with a remote team.

Responsibilities

• Conduct in-home assessments and functional evaluations.
• Deliver personalized home modification recommendations.
• Educate clients on fall prevention and safe mobility.
• Engage clients and families in collaborative care planning.
• Work with Director of Clinical Operations to share findings and support process improvement.
• Maintain accurate documentation through the electronic system.
