Job summary

Jukebox Health is seeking an Occupational Therapist to perform in-home assessments and provide personalized recommendations to enhance safety and independence for older adults and individuals with disabilities. The role supports clients and families through collaborative care planning and education on fall prevention and safe mobility.

The position is contract-based with at least 10 hours per week, and compensation is based on completed evaluations.

Qualifications

OT degree from an accredited program.
Active OT license in good standing.
At least 2 years of clinical experience.
Experience in home health or aging in place preferred.
Strong interpersonal and clinical reasoning skills.
Reliable transportation and valid driver’s license.
Proficient with technology and digital documentation.
Ability to work independently while collaborating with a remote team.

Responsibilities

Conduct in-home assessments and functional evaluations.
Deliver personalized, evidence-based recommendations for home modifications.
Educate clients and caregivers on fall prevention and safe mobility techniques.
Engage clients and families in collaborative care planning.
Work with the Director of Clinical Operations to share findings and support process improvement.
Maintain accurate and timely documentation through our electronic system.
