Job summary

Jukebox Health is hiring an Occupational Therapist to perform in-home assessments and provide personalized recommendations for home modifications, helping older adults and individuals with disabilities live safely at home.

The role requires an OT license, 2+ years of clinical experience, and comfort with technology and remote collaboration. Contracted position with at least 10 hours weekly; pay per evaluation and monthly settlements.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Active, unencumbered OT license.
• 2+ years of clinical experience.
• Experience in home health, aging in place, or disability services preferred.
• Strong interpersonal and clinical reasoning skills.
• Reliable transportation and valid driver’s license.
• Proficiency with technology and digital documentation.
• Ability to work independently while collaborating with a remote team.

Responsibilities

• Conduct in-home assessments and functional evaluations.
• Deliver personalized, evidence-based recommendations for home modifications.
• Educate clients and caregivers on fall prevention and safe mobility techniques.
• Engage clients and families in collaborative care planning.
• Work closely with Director of Clinical Operations to share findings and support process improvement initiatives.
• Maintain accurate and timely documentation through our electronic system.
