Responsibilities

• Comply with all state‑mandated Occupational Therapy Practice Act, Occupational Therapy Code of Ethics and other legal mandates.
• Evaluate client for therapy needs and provide written orders/treatment plan for services.
• Ensure plan of care is consistent with client assessment and needs.
• Instruct other personnel and/or family members and caregivers on intervention goals.
• Report client’s response to treatment or changes in condition to DON, ADON and/or client’s primary provider.
• Provide in‑service education for agency personnel, as needed.
• Ensure personal credentials are active and up to date at office.
• Attend all staff meetings and complete all required annual education as assigned.

Qualifications

• Current unrestricted Occupational Therapist license in the state for which care is being provided.
• One (1) year experience as occupational therapist, preferably in an in‑home setting.
• CPR Certification, required.
• Two‑step negative TB skin test or chest x‑ray, required.
• Valid driver’s license and auto insurance, required.
• Understands regulatory compliance with state and federal regulations.
• Adheres to HIPAA and maintains client confidentiality.
• Transportation to meet the needs of position and scope of job responsibilities, required.
