Job summary

Pathways Healthcare is seeking a compassionate Licensed Practical Nurse (LPN) to join our in-home care team serving the Boston area from our Westwood, MA office. You will provide direct patient care, collaborate with RNs and physicians, and support families to achieve positive outcomes.

The role offers a per diem schedule, competitive per visit rate, mileage reimbursement, and a focus on patient-centered care in a supportive work environment.

Qualifications

• Massachusetts LPN license required.
• Graduate of an accredited practical nursing program.
• Home health or hospice experience preferred.
• BLS CPR certification required.
• Valid driver’s license and insured vehicle.

Responsibilities

• Provide skilled nursing care under RN supervision.
• Monitor and document patient condition and responses to treatment.
• Administer medications and treatments as prescribed.
• Assist with implementation and evaluation of care plans.
• Educate patients and caregivers on medications and disease management.
• Communicate changes in patient condition to RN and physician.
• Maintain accurate clinical documentation.
• Promote a safe, comfortable environment for patients.
• Coordinate supplies, equipment, and services for patient care.
• Support quality improvement initiatives and compliance standards.
• Accept and fulfill assigned patient visits and schedules.
