Pathways Healthcare in Worcester, MA is seeking a Nurse Practitioner to provide in-home care across Worcester, Sturbridge and surrounding areas. This per diem role offers flexible scheduling and the chance to impact patients’ recovery in a home setting.

Responsibilities include admissions assessments, treatment planning, wound care, medication management, discharge planning, and collaboration with the care team while maintaining confidentiality.

Qualifications

• Current APRN state professional license (Adult/Geriatrics specialty preferred), DEA certificate, CPR certification.
• Master’s degree in Advanced Practice Nursing from an accredited school.
• At least 2 years as a Nurse Practitioner, preferably in home health care.
• Valid driver’s license with reliable transportation.

Responsibilities

• Conduct admission assessments and develop personalized treatment plans.
• Provide routine and urgent care visits, ensuring flexibility to meet patient needs.
• Manage medication orders and educate patients on proper usage.
• Handle wound care, testing, and equipment/supply needs.
• Document patient progress and collaborate with the care team for optimal results.
• Assist in discharge planning and coordinate with community resources.
• Participate in team meetings and briefings to ensure cohesive patient care.
• Maintain patient confidentiality and report any incidents or safety concerns.
