Job summary

Signify Health seeks an in-home clinician to perform visits for health plan members, reviewing medical history and conducting evaluations with a focus on diagnostic and preventive services. You’ll work within a national network, traveling locally and across licensed states as needed, using an iPad and standard clinical workflow.

This is a full-time role with comprehensive benefits. Applicants must hold an active, unrestricted license in relevant coverage areas and board certification;

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.

• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
• Be flexible to travel locally and within licensed states, as needed.
• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.)
