Job summary

Optum Care at Home seeks a Nurse Practitioner to deliver comprehensive in-home care to high-risk patients as part of an interdisciplinary team, aiming to prevent hospitalizations and improve quality of life.

This role offers autonomy, strong patient relationships, chronic disease management, and significant statewide travel with weekends at home, traveling across assigned states including AL, AR, CO, CT, GA, IA, MO, NC, NY, OH, OK, PA, SC, WA, WI.

Qualifications

• Active ANCC or AANP certification (Family, Adult, Geriatric, or Adult-Gerontology).
• Active, unrestricted state NP license.
• Active driver's license and reliable transportation.
• DEA license/prescriptive authority (or ability to obtain).
• Ability to obtain collaborative practice agreement if required by state law.
• Ability to travel extensively and perform home visits in various settings.

Responsibilities

• Conduct comprehensive in-home assessments.
• Medical history review and medication reconciliation.
• Physical examinations, vital signs and screenings.
• Manage acute illnesses and chronic condition exacerbations.
• Provide care during short-term rehabilitation episodes.
• Document diagnoses and ICD-10 conditions accurately.
• Collaborate with interdisciplinary care teams.
• Lead advance care planning conversations.
• Educate patients and caregivers on treatment plans and disease management.
• Coordinate care and support services to improve outcomes.
