HarmonyCares is seeking a Nurse Practitioner to perform in-home health risk assessments and telehealth visits for patients with complex care needs. You will evaluate health conditions, document findings, and coordinate with physicians to ensure comprehensive care in the home setting.

Ideal candidates hold an active NP license, board certification, BLS, and strong EHR skills. Ability to travel within multiple states and maintain patient confidentiality is essential.

Competenze

• Active/unrestricted nurse practitioner license to practice in coverage states.
• Board certification in ANCC, AANP or NCCPA.
• Active BLS Certification.
• Current enrollment in Medicare/Medicaid.
• Valid and unencumbered driver's license with good driving record.
• Outstanding EHR skills.

Mansioni

• Conduct in-home health risk assessments and identify active and chronic conditions.
• Take patient history, exam, and determine diagnoses.
• Provide written documentation of patient visits per NCQA standards.
• Measure vitals and perform essential clinical tasks.
• Place case management referrals and coordinate with PCPs.
