HarmonyCares is seeking a Nurse Practitioner to perform in-home risk assessments and collaborate with a physician-led team. The role includes visiting patients in residential settings or via telehealth, documenting findings, and coordinating with PCPs and care teams.

Qualified candidates hold an active NP license with board certification and BLS, plus strong EHR skills. Travel to patient homes may be required; no holidays and comprehensive benefits are offered.

Competenze

• Active/unrestricted nurse practitioner license in coverage states.
• Board certification ANCC/AANP or NCCPA.
• Active BLS Certification.
• Current enrollment in Medicare/Medicaid.
• Valid driver's license with good driving record.
• Outstanding EHR skills.

Mansioni

• Conduct in-home health risk assessments to identify active and chronic conditions and social needs.
• Takes history, examines, determines diagnoses.
• Provides written documentation of patient visit per NCQA standards.
• Takes patient vital signs as necessary.
• Places case management referrals and communicates with PCP as needed.
• Communicates with patients, caregivers, nurses, providers and vendors to assure proper diagnosis.
• Performs clinical duties while observing OSHA precautions.
• Maintains patient confidentiality.
• Attends required meetings and in-services and participates in committees.
• Participates in professional development and maintains licenses.
• Performs other job-related assignments as requested.
