Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. You’ll engage with people in the comfort of their own homes, review medical history and medications, perform a focused physical evaluation, and help bridge gaps in care.

This part-time role involves local travel, virtual visits as needed, and a base hourly pay range of $46.03–$99.14 with eligibility for benefits.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.
• Part-time, per diem schedule with ~12 hours weekly.

Responsibilities

• Conduct in-home health evaluations.
• Review medical history and medications; perform physical evaluation.
• Travel locally and within licensed states; provide virtual visits as needed.
