Job summary

Signify Health, part of CVS Health, is seeking per diem Nurse Practitioners or Physician Assistants to provide in-home health evaluations. Clinicians engage with patients at home, reviewing medication histories and performing physical exams in a flexible, patient-centered setting.

You'll join a national network of clinicians, travel to homes within licensed states, and conduct virtual visits as needed. Per diem pay includes an hourly base with incentives and comprehensive benefits for eligible

Qualifications

• Active, unrestricted license(s) in coverage areas.
• Board certification (required).
• Multi-state licenses (preferred).

Responsibilities

• Provide In-Home Health Evaluations for health plan members.
• Engage with patients in their homes to assess health status.
• Perform medication and medical history review, physical evaluation.
• Coordinate with Signify Health on diagnostic & preventive services.
• Travel locally and within licensed states as needed.
• Conduct virtual visits when required.
