The role offers part-time hours, with flexible local travel and a base hourly rate ranging from $37.66 to $81.11, plus incentives and benefits for part-time staff.

Qualifications

• Active, unrestricted license(s) in coverage areas.
• Board certification is required.
• Multi-state licenses are preferred.

Responsibilities

• Provide in-home health evaluations during visits lasting about 45 minutes.
• Engage with patients to address health questions and bridge gaps in care.
• Use iPad and clinical workflow to conduct evaluations; no prescribing or altering treatment.
