Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Clinician (NP or PA) to perform in-home health evaluations for members in their own homes. The role involves interviewing patients, reviewing medications and medical history, and conducting a physical evaluation with optional diagnostic tests as ordered by health plans.

The position offers a part-time schedule with ~12 hours per week, per-visit compensation, and benefits.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

Responsibilities
