Job summary

Signify Health, part of CVS Health, is seeking a Part-Time Clinician (NP or PA) to perform in-home health evaluations for members. The visits last about 45 minutes and focus on health questions, medication review, and brief physical assessments in the patient’s home.

You will travel locally and within licensed states, use an iPad with a clinical workflow, and may conduct virtual visits as needed. A current, unrestricted license, board certification, and multi-state credentials are required or

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

Responsibilities

• Provide In-Home Health Evaluations for health plan members in their homes.
• Engage with people during visits to understand health needs and bridge gaps in care.
• Use an iPad and clinical workflow; no required prescribing or lab orders.
• Be flexible to travel locally and within licensed states; conduct virtual visits as needed.
• Provide Diagnostic & Preventive Services as indicated, per health plan orders.
