Job summary

Signify Health, part of CVS Health, is seeking a Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations for members in their own homes. You will conduct medication and medical history reviews, physical evaluations, and, when ordered by the health plan, additional diagnostic tests.

Travel locally within licensed states is required; virtual visits are also supported.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).
• Able to cover Livingston, Rochester and surrounding area

Responsibilities

• Conduct in-home health evaluations for health plan members.
• Use iPad and clinical workflow for evaluations.
• Travel locally to licensed states; conduct virtual visits as needed.
• Provide diagnostic and preventive services as indicated (e.g., spirometry).
