Job summary

Signify Health, part of CVS Health®, seeks a Part Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations at members’ homes, bridging gaps in care.

The role involves ~29.5 hours weekly, with a national travel footprint and virtual visit options, using an iPad for a streamlined workflow. Board-certified NPs or PAs with active licenses are required.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

• Conduct virtual visits as needed.
• Provide all Signify Health services including Diagnostic & Preventive Services (e.g., spirometry, DEE, bone density screening).
