Job summary

Signify Health, part of CVS Health, is seeking a Part‑Time Clinician (NP or PA) to perform in‑home health evaluations. The role involves visiting patients at home, reviewing medical histories, performing physical assessments, and coordinating with health plans.

Must hold an active unrestricted license, board certification, and may require multi-state credentials. Travel locally within licensed states is expected.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
