Job summary

Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. This in‑home clinical role involves 40 hours per week with local travel within licensed states.

Candidates must hold an active NP/PA license, board certification; multi‑state licenses are preferred.

Qualifications

• Active, unrestricted license(s) in coverage area(s) is required.
• Board certification is required.
• Multi-state licenses are preferred but not mandatory.

Responsibilities

• Provide in-home health evaluations to patients in their homes.
• Engage with patients to address health concerns and bridge gaps in care.
• Utilize iPad workflow for efficient assessments and documentation.
• Travel locally to visit patients within licensed states as needed.
