Job summary

Signify Health, Inc. is seeking a Part Time Clinician (NP or PA) to provide in-home health evaluations in patients' homes, bridging gaps in care and linking members to the right services.

The role involves up to 29.5 hours per week with travel locally and within licensed states, using an iPad and clinical workflow. You will evaluate and discuss health plans, conduct virtual visits as needed and support Diagnostic & Preventive Services. Benefits and flexible scheduling offered.

Qualifications

• Active, unrestricted licenses in the coverage areas.
• Board certification is required.
• Multi-state licenses are preferred.

• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
