Job summary

Signify Health conducts in-home health evaluations, visiting patients in their own homes to bridge care gaps and improve outcomes. Clinicians use iPads and a streamlined workflow to perform histories, med reviews, and physicals, without prescribing or ordering labs unless required by health plans.

The role offers part-time hours (~29.5 per week) with flexible travel to licensed states and potential for virtual visits as needed, along with a comprehensive benefits package for part-time employees.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.
• Able to cover Denver County.

Responsibilities

• Engage with patients during in-home visits, providing clinical evaluations.
• Perform history and medication review during visits.
• Conduct physical examinations as needed and follow clinical workflows.
• Coordinate with health plans and Signify Health for tests (when ordered).
• Offer virtual visits when required and travel locally within licensed states.
