Job summary

Signify Health is seeking a part-time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations, visiting members at their homes and bridging gaps in care. The role emphasizes compassionate patient interactions and flexible scheduling within licensed states.

Clinicians will use an iPad with a streamlined workflow and will engage in diagnostic and preventive services as indicated.

Qualifications

• Active, unrestricted licenses in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

Responsibilities

• Perform in-home health evaluations for plan members.
• Engage with patients, address questions, and bridge gaps in care.
• Use iPad and clinical workflow during visits; no prescribing or lab orders required.
• Travel locally within licensed states; conduct virtual visits as needed.
• Provide Signify Health services such as diagnostic and preventive screening.
