Job summary

Signify Health, part of CVS Health, seeks Full-Time Clinicians (NPs or PAs) to perform in-home health evaluations for health plan members in their homes. You will conduct comprehensive assessments using a streamlined digital workflow and may travel to licensed states as needed.

This role requires active, unrestricted licenses and board certification; multi-state licenses are preferred. The position offers a full benefits package and stability for a national network of clinicians.

Qualifications

• Active, unrestricted licenses in coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.

Responsibilities

• Provide in-home health evaluations for health plan members.
• Engage with patients and bridge gaps in care.
• Conduct assessments using iPad-based workflow.
• Travel locally and within licensed states as needed.
• Possibly conduct virtual visits when required.
• Provide Diagnostic & Preventive Services as indicated.
