Job summary

Signify Health, part of CVS Health, seeks a Part-Time Clinician (NP/PA) to deliver In-Home Health Evaluations, visiting members in their own homes and bridging gaps in care. Visits last about 45 minutes, including medication and medical history review, a physical exam, and optional tests if required by the plan.

You will join a national clinician network, travel locally within licensed states, and may conduct virtual visits as needed.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.

Responsibilities

• Provide In-Home Health Evaluations for health plan members.
• Visits last about 45 minutes, with medication and medical history review, physical evaluation, and optional diagnostic tests.
