Job summary

VNA Health Group's Visiting Physician Services (VPS) seeks a Nurse Practitioner to deliver high‑quality, home‑based primary care to older adults in New Jersey. You will assess, diagnose, and manage care for homebound patients in collaboration with an interdisciplinary team.

The role requires NJ APN licensure with prescriptive authority, national NP certification, 2+ years of APN experience, and strong communication and computer skills.

Qualifications

• Master’s degree in Nursing from an NLN accredited program.
• NJ Advanced Practice Nurse licensure with prescriptive authority; CDS and DEA.
• National certification as an Adult or Geriatric Nurse Practitioner.
• 2+ years APN experience or significant RN background.
• Excellent communication, organizational and interpersonal skills.
• Strong computer skills.

Responsibilities

• Assesses, diagnoses and develops plan of care for homebound patients.
• Monitors patient condition and response to treatment.
• Acts as agency representative with patients, families and other health agencies.
• Participates in organizational QAPI programs and in‑service trainings.
• Adheres to infection control and safety policies.
• Maintains appropriate appearance and punctuality.
