Job summary

Signify Health, part of CVS Health®, seeks Per Diem Clinicians (NPs or PAs) to perform in-home health evaluations. Responsibilities include comprehensive history reviews, physicals, and optional diagnostic tests, with visits typically lasting around 45 minutes and online workflow via iPad.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.

Responsibilities

• Visit patients in their homes for health evaluations.
• Engage with members to review medical history and perform physical evaluations.
• Utilize Signify Health clinical workflow and iPad for data collection.
• Coordinate with health plans and clinicians to connect members with appropriate care.
• Conduct virtual visits as needed and travel locally as required.
