Job summary

Northeast Healthcare Recruitment, Inc. (NEHCR) seeks a Nurse Practitioner to perform in-home health assessments across Maine on a flexible, per diem basis. The role includes reviewing medical histories, conducting exams, and screening for chronic conditions, with reimbursement for travel and mileage.

You will educate patients on medication management, disease prevention, and overall health maintenance, and coordinate findings with primary care providers to ensure continuity of care.

Qualifications

• Active Nurse Practitioner license in Maine.
• Board Certification from AANP or ANCC in Family, Adult-Gerontology, or related specialties.
• Minimum of 1 year of clinical experience; geriatric experience preferred.
• Strong communication and interpersonal skills with patient-centered care focus.
• Reliable transportation for local travel and comfort with varied home environments.

Responsibilities

• Perform in-home health assessments including reviewing medical histories and physical exams.
• Educate patients on medication management, disease prevention, and health maintenance.
• Document diagnoses and communicate findings for care management.
• Collaborate with primary care providers to address gaps in patient care.
