Qualifications

• Active, unrestricted NP license is mandatory.
• Board certification required: FNP, AGNP, or ANP.
• Reliable personal transportation is essential.

Responsibilities

• Complete pre-scheduled, in-home health assessments.
• Work with pre-populated patient forms for efficiency.
• Ensure a minimum of 60 hours/month commitment.

Skills
