Job summary

Signify Health, part of CVS Health, seeks a Full Time Clinician (NP or PA) to provide in-home health evaluations for plan members, visiting patients in their homes and bridging gaps in care.

You'll use an iPad with a clinical workflow, perform history reviews and physicals, and coordinate with health plans. Travel locally within licensed states; virtual visits as needed; comprehensive benefits with standard medical, dental, vision, PTO, retirement.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification.
• Multi-state licenses preferred.
• Able to cover the following primary locations.

Responsibilities

• Provide in-home health evaluations at patients' homes.
• Build trusted relationships to improve health outcomes.
• Travel locally within licensed states; conduct virtual visits as needed.
• Use iPad and clinical workflow to perform evaluations; no prescribing required.
• Coordinate with health plans to connect members to appropriate care.
