Qualifications

• Active, unrestricted NP/PA license in Minnesota or eligible in-state licensing.
• Board certification as NP or PA is required.
• Post-graduate collaborative practice hours completed (2080 hours) within integrated clinical care setting.

Responsibilities

• Conduct in-home health evaluations for members.
• Summarize health findings and bridge gaps to appropriate care.
• Utilize iPad and clinical workflow for evaluations.
• Travel locally within licensed states and conduct some virtual visits.
