Job summary

Hueman is seeking licensed Nurse Practitioners to perform in-home Health Risk Assessments for Medicare/Medicaid members around Boise, ID. This role offers flexible scheduling and the chance to impact patient care in a home setting.

Responsibilities include travel to patient homes, performing non-invasive assessments, reviewing medical history, documenting findings, and providing basic wellness education. Active NP license and BLS are required; Medicare ID and board certification preferred.

Qualifications

• Master’s Degree OR commensurate experience and NP license.
• Active State NP license.
• Medicare ID #.
• Active National board certification.
• Current BLS certification.

Responsibilities

• Travel to and from patients' homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure documentation.
• Conduct medical reconciliation for patient safety and compliance.
• Provide basic patient education on wellness and preventative care.
