Job summary

Hueman seeks Nurse Practitioners to perform in-home Health Risk Assessments for Medicare/Medicaid members around Multnomah County, Oregon. This role offers flexible scheduling and travel reimbursement, with comprehensive home assessment training and scheduling support.

Qualified NPs will hold an active state license, national board certification, and BLS. Prior paid clinical experience is preferred. You will visit patients in their homes, documenting findings and educating on wellness measures.

Qualifications

• Master’s degree in nursing or related field.
• Active State NP license and national board certification.
• BLS certification is required or active.
• 12 months of paid advanced clinical provider experience preferred.
• Ability to travel to patients’ homes for HRAs.

Responsibilities

• Travel to and from patients’ homes for assessments.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and document findings accurately.
• Conduct medical reconciliation to ensure safety and compliance.
• Provide basic education on wellness and preventive care.
