Hueman is seeking Nurse Practitioners to conduct in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in the Lyndonville, VT area. This role offers flexible scheduling, travel reimbursement, and comprehensive home assessment training, with dedicated patient scheduling support and liability insurance options.

Nurse Practitioners will perform non-invasive assessments, review medical history, and ensure proper documentation.

Qualifikationen

• Master’s Degree in Nursing or commensurate experience with NP license.
• Active State NP license.
• Active national board certification.
• Current BLS certification.
• 12 months of paid advanced clinical provider experience preferred.

Aufgaben

• Travel to patients’ homes to perform HRAs.
• Conduct comprehensive, non-invasive physical assessments.
• Review medical history and document promptly.
• Perform medical reconciliation for patient safety and compliance.
• Provide basic wellness education to patients.
