Job summary

Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Brattleboro, VT. This role involves traveling to patients’ homes and performing comprehensive, non-invasive assessments without prescribing in home.

The position offers a flexible schedule and an opportunity to impact community health. Qualified NPs will have an MSN/NP credential, an active state license, national board certification, and current BLS.

Qualifications

• Master’s degree in nursing or equivalent.
• Active state NP license.
• Active National Board Certification as NP.
• Current BLS certification.
• 12 months of paid advanced clinical provider experience preferred.

Responsibilities

• Travel to and from patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative healthcare measures.

Education

Master’s Degree in Nursing (MSN) / NP credential

State NP license

National Board Certification (NP)

BLS certification

12 months of paid advanced clinical provider experience preferred
