Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Blue Ridge, GA. Qualified NPs will interact with members in intimate settings, perform non‑invasive assessments, and document findings thoroughly.

Qualifications

• Must hold an active State NP license and national board certification.
• BLS certification is required at the time of hiring.
• Masters degree in nursing or commensurate experience with NP qualification.
• Minimum 12 months of paid advanced clinical provider experience preferred.

Responsibilities

• Travel to and from patients' homes to perform HRAs.
• Conduct comprehensive, non-invasive physical assessments and review medical history.
• Document findings accurately and perform medical reconciliation.
• Provide basic patient education on wellness and preventative care.
