Signify Health, part of CVS Health®, is seeking a Full-time Nurse Practitioner for in-home health evaluations. The role involves visiting patients at their homes, conducting comprehensive assessments, and bridging gaps in care across licensed states.

Qualifications

• Active, unrestricted NP license in coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.

Responsibilities
