Job summary

Signify Health, part of CVS Health, seeks a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations, engaging with members in their own homes and bridging gaps in care. The visit lasts about 45 minutes and includes review of medications and history, a physical evaluation, and optional tests as ordered by the health plan.

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).
