Job summary

Signify Health, part of CVS Health, seeks a Full Time Clinician (NP or PA) to provide in-home health evaluations. You will travel up to 100% across defined coverage areas, performing comprehensive visits in members' homes with a focus on bridging gaps in care.

The role emphasizes clinical assessment, history review, and physical evaluation, using an iPad and standardized workflow. Requires multiple state licenses and board certification.

Qualifications

• Active, unrestricted NP or PA license in coverage area.
• Board certification required.
• Able to obtain multi-state licenses as needed.
• Able to cover all counties within state and flex to other areas of business need.

Responsibilities

• Provide in-home health evaluations for members.
• Complete medication and medical history review.
• Perform physical evaluations as part of visits.
• If ordered by the health plan, conduct diagnostic tests (e.g., spirometry).
• Travel locally and within licensed states as required.
• Utilize iPad and clinical workflow during visits.
