Job summary

Advantmed is seeking a licensed NP or PA to conduct structured, in-home wellness and risk adjustment assessments for diverse patient populations. This PRN role is 1099 independent contractor with flexible weekdays and no evenings or weekends.

You'll perform annual wellness visits, chronic condition assessments, and accompanying screenings, documenting visits in the EMR to support value-based care. Expect reliable, ongoing income with strong operational support.

Qualifications

• Active, unencumbered NP or PA license in the applicable state.
• 3+ years of patient care experience preferred.
• Willing to obtain additional licensure if needed (supported by Advantmed).

Responsibilities

• Conduct in-home Annual Wellness Visits and chronic condition assessments
• Perform focused histories, physical exams, vitals, medication reviews, and screenings
• Educate members on preventive care and chronic disease management
• Document visits in the EMR to support value-based care and risk adjustment
• Operate within NP or PA scope of practice and state regulatory guidelines
