Job summary

Advantmed is seeking licensed NP or PA clinicians to conduct structured, in-home wellness and risk adjustment assessments under a 1099 PRN arrangement. Expect reliable scheduling, autonomy, and solid operational support with visits around a ~55-mile radius of your assigned county.

Typical duties include comprehensive histories, exams, labs/composite screenings, and EMR documentation to support value-based care.

Qualifications

• Active, unencumbered NP or PA license in the applicable state.
• 3+ years of patient care experience preferred.
• Willing to perform in-home assessments and travel within ~55 miles as needed.

Responsibilities

• Conduct in-home Annual Wellness Visits and chronic condition assessments.
• Perform focused histories, physical exams, vitals, medication reviews, and screenings.
• Document visits in the EMR to support value-based care and risk adjustment.
• Operate within NP or PA scope of practice and state regulatory guidelines.
