Signify Health, part of CVS Health, is seeking a Part-Time Clinician (NP or PA) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes.

You’ll travel locally within licensed states, perform medication and medical history reviews, and conduct physical evaluations using an iPad workflow, with virtual visits as needed.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

Responsibilities

• Engage patients in their homes through In-Home Health Evaluations and bridge gaps in care.
• Travel locally to visit patients across licensed states.
• Conduct virtual visits as needed.
• Perform diagnostic and preventive services as indicated by health plans.
