Job summary

Signify Health, part of CVS Health, seeks a Part Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations for plan members, engaging with them in their own homes.

You will travel locally, conduct visits about 45 minutes, review medications and medical history, perform a physical evaluation, and provide diagnostic and preventive services as indicated.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

Responsibilities

• Bring your clinical expertise to in-home visits and build trust with health plan members.
• Conduct pre-visit planning and health evaluations with a focus on patient safety and comfort.
• Perform physical evaluations and review medications and medical history at the patient’s home.
• Deliver DI/Preventive Services as ordered by the health plan and use Signify Health workflows.
• Travel locally and within licensed states as needed; conduct virtual visits when appropriate.
• Collaborate with Signify Health clinicians to ensure comprehensive patient care.
