Optum Care at Home in Thurston County, WA is seeking a Nurse Practitioner or Physician Assistant to provide longitudinal, home-based care for high-risk patients with complex medical needs. The role expects collaboration with interdisciplinary teams to improve quality of life and manage chronic illnesses.

Required credentials include ANCC/AANP for NP or NCCPA for PA, active licensure and DEA, with a valid driver's license and ability to work independently across home-based settings.

Qualifications

• NP: ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• PA: NCCPA certification and active state licensure
• Active, unrestricted license (or ability to obtain)
• Active DEA license/prescriptive authority (or ability to obtain)
• Valid driver's license and reliable transportation
• Ability to work independently in various home-based care settings

Responsibilities

• Conduct comprehensive in-home assessments, including physical exams, medication reviews, screenings, and diagnostics
• Manage acute illnesses and chronic disease exacerbations
• Provide advanced care planning and end-of-life discussions
• Educate patients and caregivers
• Ensure accurate ICD-10 documentation and coding
• Collaborate closely with interdisciplinary care teams and other providers
