Job summary

Optum HouseCalls in Mitchell, Yancy and Avery Counties, NC is seeking a Nurse Practitioner or Physician Assistant to perform annual in-home health assessments for Medicare Advantage and other health plan members.

This field-based, non-prescribing role focuses on preventive care, patient education, and improving health outcomes through collaboration with primary care providers and the care team.

Qualifications

• Active, unrestricted licensure in the practicing state (or ability to obtain by start date).
• Active and unrestricted driver\'s license.
• Reliable transportation to complete home visits.
• Ability to navigate varied home environments and perform assessments.
• Compliance with licensing requirements and board approvals when applicable.

Responsibilities

• Conduct comprehensive in-home health assessments.
• Identify and close gaps in care for members.
• Educate patients on disease management, medications, and preventive care.
• Collaborate with primary care providers and interdisciplinary care team.
• Address social determinants of health and connect members with resources.
