Qualifications

• Active licensure in practicing state with no disciplinary actions
• Driving/transportation access required for home visits
• Ability to navigate varied home environments and perform assessments

Responsibilities

• Conduct comprehensive in-home health assessments for Medicare Advantage and other members
• Identify and close gaps in care with primary care providers
• Educate patients on disease management, medications, and preventive care
• Address social determinants of health and connect members with resources
• Collaborate with an interdisciplinary care team to support member health
