Job summary

Signify Health, part of CVS Health, seeks a Full Time Clinician (NP or PA) to perform in-home health evaluations across a defined coverage area. You will travel up to 100% of the time, depending on business needs, and may require overnight stays and multi-state licenses.

You will connect with members, review medical history, perform physicals, and provide diagnostic and preventive services per plan. Primary workplace is home visits.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required.
• Able to obtain and maintain multi-state licenses as needed.
• Able to cover all counties within state and flex to other areas of business need.

Responsibilities

• Provide in-home evaluations and health assessments.
• Engage with health plan members to bridge gaps in care.
• Conduct visits across defined coverage area, including travel.
• Perform diagnostic and preventive services as indicated in care plan.
• Coordinate with Signify Health clinicians to ensure quality visits.
• Utilize iPad and clinical workflow without changing current treatments.
