Job summary

Signify Health, part of CVS Health, is seeking a Full Time Clinician (NP or PA) to perform in-home health evaluations across a defined national coverage area. The role requires travel up to 100% and may include overnight travel; licenses across states and board certification are required.

Visits are conducted in patients' homes, typically lasting about 45 minutes, with comprehensive evaluations and optional tests per health plan orders. A supportive team and benefits are provided.

Qualifications

• Active, unrestricted licenses in the coverage area(s) you will serve.
• Board certification is required for NP/PA roles.
• Able to obtain and maintain multi-state licenses as needed.
• Must cover all counties within the state and flex to other areas as needed.

Responsibilities

• Join a national network of clinicians dedicated to improving health outcomes.
• Visit millions of people nationwide to connect them with appropriate care.
• Collaborate as a clinician with Signify Health on assigned duties.
• Be flexible to travel locally and within licensed states.
• Conduct virtual visits when required.
• Provide Diagnostic & Preventive Services such as spirometry and screenings.
